LSO Claim Form

Claims involving missing or late packages must be submitted within 30 days. Claims for damaged packages must be received within 15 days. All packaging materials must be kept and remain available for inspection at any time until the claims process has been completed.

Please note that only an LSO account holder is eligible to file a claim. If you are expecting a package and need to file a claim, please reach out to your vendor for further assistance. To create your free LSO account, click here.
